چکیده

FLUIDS ARE A CORE ELEMENT IN the resuscitation of critically ill patients and the relative superiority and safety of different resuscitation solutions has been the focus of considerable debate. Crystalloid solutions are inexpensive and readily available, while colloid solutions may minimize resuscitation volumes, and may sustain intravascular volume for longer durations. Conflicting results from clinical trials and systematic reviews have not resolved this debate, leaving clinicians to select resuscitation fluids based on suboptimal evidence. Hydroxyethyl starch is a synthetic colloid derived from partially hydrolyzed and variably hydroxyethylated plant starch, and is commonly administered to patients requiring fluid resuscitation. Clinical trials and systematic reviews have suggested a greater incidence of renal damage and mortality in patients receiving hydroxyethyl starch, but these findings have been inconsistent. The differing results may be due to a combination of factors including differing patient populations,

Association of hydroxyethyl starch administration with mortality and acute kidney injury in critically ill patients requiring volume resuscitation: a systematic review and meta-analysis.

IMPORTANCE Hydroxyethyl starch is commonly used for volume resuscitation yet has been associated with serious adverse events, including acute kidney injury and death. Clinical trials of hydroxyethyl starch are conflicting. Moreover, multiple trials from one investigator have been retracted because of scientific misconduct. OBJECTIVES To evaluate the association of hydroxyethyl starch use with...

Renal outcomes and mortality following hydroxyethyl starch resuscitation of critically ill patients: systematic review and meta-analysis of randomized trials

BACKGROUND Hydroxyethyl starch (HES) is a type of colloid fluid that is commonly used for volume resuscitation of patients admitted to the intensive care unit. Data regarding the renal consequences of HES are conflicting. PURPOSE To evaluate the effect of HES solutions on renal outcomes and mortality among critically ill patients requiring acute volume resuscitation. DATA SOURCES We searche...

Fluid resuscitation with 6% hydroxyethyl starch (130/0.4) in acutely ill patients: an updated systematic review and meta-analysis.

BACKGROUND Recent research suggests that 6% hydroxyethyl starch (HES) 130/0.4 is one of the most frequently used resuscitation fluids worldwide. The retraction of studies evaluating its use necessitates a reevaluation of available evidence regarding its safety and efficacy. METHODS We performed a systematic review and meta-analysis of unretracted randomized controlled trials comparing the eff...

The effect of hydroxyethyl starches and crystalloid fluids on mortality and need for dialysis in vascular surgery patients

Introduction Previous research has indicated that fluid choice is an important component of the resuscitation of the critically-ill patient. Synthetic hydroxyethyl starches (HES) may remain within the intravascular space compared to crystalloid solutions, yet they have also been associated with acute kidney injury. Data comparing HES and crystalloid solutions in anesthesia is incomplete. The go...

Pharmacokinetic Behavior of Theophylline following PEEP in Critically Ill Patients with Acute Lung Injury

The effect of Positive End Expiratory Pressure (PEEP) on the hepatic elimination of low to moderate extraction ratio drugs has not been clearly defined. We prospectively investigated the effect of PEEP on the clearance of theophylline in 30 (20 males and 10 females) intubated critically ill adult patients with acute lung injury/acute respiratory distress syndrome (ALI/ARDS). The Mean (±SD) age ...
